Support Broker Services, LLC, in collaboration with Forward Health, is seeking a compassionate and skilled Registered Nurse to join our team in delivering high-quality, personalized care to patients in their homes. Your Responsibilities:

  • Deliver specialized care during flexible 12-hour shifts (daytime: 7 AM – 7 PM or overnight: 7 PM – 7 AM).
  • Implement advanced seizure management protocols and behavioral support strategies.
  • Provide targeted care for patient with **Lennox-Gastaut Syndrome (LGS)** and other treatment-resistant epilepsy conditions.
  • Collaborate with a multidisciplinary team to ensure patient safety, comfort, and improved quality of life.


Qualifications:

  • Active Wisconsin Registered Nurse (RN) license.
  • Proven experience in seizure management, behavioral health, and caring for medically complex patients.
  • Compassionate, patient-centered approach with strong critical thinking skills.
