Your Specific Responsibilities:

As part of the Community Health Planning, Promotion, and Communication Team, the Health Education Program Consultant will improve community health by participating in the planning, implementation, and evaluation of programs and activities designed to encourage healthy behaviors through individual, policy, system, and environmental change. This position is responsible for conducting the full range of activities required to prepare, submit, and manage grant proposals to foundation and corporate sources. This includes researching, compiling, writing, and editing grant applications, as well as developing templates to streamline the grant proposal process. This position requires the individual to be self-motivated and work independently with minimal supervision. Hours will be flexible to accommodate after hours/weekends needs of the community.

 

Grant Management :

  • Understanding chronic disease risk factors and barriers to health in our community.
  • Employing evidence-based practices to reduce or prevent the risk factors that contribute to chronic disease.
  • Using community-oriented, participatory models for creating successful community health promotion initiatives.
  • Planning and coordinating the use of educational resources to motivate and assist individuals and groups to adopt and maintain healthful practices and lifestyles.

 

Planning and Reporting:

  • Ensures plan requirements are in met and in compliance and submissions are on time.
  • Reviews and analyzes activities and data to aid in planning, resource utilization, and provision of services.
  • Assists with community development initiatives, including marketing, programming, and service offerings.
  • Collects data needed to complete and submit reports in accordance with program requirements.

 

Health Promotion:

  • Engages in outreach initiatives aimed at improving health and enhancing the quality of life for residents.
  • Identifies and establishes collaborations with key partners serving underserved populations to disseminate accurate information and facilitate access to health-related services.
  • Collaborates across sectors to improve processes, practices, and policies aimed at advancing health promotion initiatives.
  • Attends trainings related to community health improvement.
  • Facilitates public health education by developing, leading, and participating in workshops, seminars, community presentations, task force meetings, and other related activities.
  • Assists to meet State-mandated Health Promotion and education activities that support the DOH-Charlotte and/or the State Strategic Plans.
  • Enhances and promotes DOH-Charlotte programs.

 

Community Outreach:

  • Leads and participates in activities to support the departments multiple work plans, emphasizing community engagement.
  • Participates in the planning, preparation, and teaching of educational programming in the community.
  • Works with community organizations and partners to link individuals and families with resources and services.
  • Engages in and educates decision-makers on the initiatives of the Community Health Promotion Department.
  • Collects data needed to complete and submit reports based on program requirements.

Qualifications:

  • A valid Florida driver’s license is required to perform the duties of this position
  • A minimum of one (1) year of work experience in grant writing and management, public health, health education and/or related field is required
  • Certification in child passenger safety or the ability to obtain certification within six (6) months of hire is required
  • A bachelor’s degree from a U.S. accredited college or university is preferred
