Job Description

Hours: Full time – 40 hours (Rotating on call schedule)

Supplement Pay: Relocation and student loan forgiveness payments

Must-Haves:

• Transplant pharmacy experience
• Graduate of an ACPE-accredited school of pharmacy
• PharmD or equivalent education
• Hospital or ambulatory training/experience required based on practice area
• 2+ years of direct, relevant, clinical experience OR PGY-2 residency (preferred)
• Strong written and verbal communication skills
• Ability to interact effectively with medical, nursing and pharmacy staff, pharmacy residents and students is required
• Knowledge and understanding of general hospital and outpatient clinic operations
• Ability to integrate clinical services and educational activities
• Registered pharmacist in Florida or eligible (*Will accept out of state license)

Plusses:

• PGY-2 / PGY-3 residency preferred
• Experience in transplant outpatient ambulatory clinic
• Board certification**
• EPIC experience**
• FACT accreditation – need a certain about of CEs per year**
• HOPA affiliation
• Research from an academic organization (more common w/ Bone Marrow)
• Florida License
• ASTCT
• BPOC Certified

Day-to-Day:

• Will work closely with pharmacy team and transplant teams to make the best decisions for patients
• This position will be responsible for supporting abdominal solid organ clinical pharmacy services on an academic teaching service
• Prioritize which patients they will see in the clinic based off why they are in the clinic, how long ago they received transplant or how long before they will receive transplant, and what medicines they need managed
• Clinical pharmacists are aligned with assigned patient population, medical team or designated specialty area within the outpatient ambulatory clinic
• Provides active participation in related education and research activities.
• Assist in training resident pharmacy specialists
• Sit on medical review board that decides if patient is fit to receive a transplanted organ or not
• Condition of employment: all employees must be accessible by telephone. This requires each employee to provide a valid telephone number to the department staff assistant in order for the department to reach them in the event of hospital need and/or emergency.
