The Yale Hypertrophic Cardiomyopathy program is seeking a qualified Genetic Counselor interested in working in a growing program that includes a physician leader specializing in cardiomyopathies that have a hereditary component. The mission of the program is to provide outstanding integrated patient care while keeping the academic commitment to teaching and performing research in different aspects of inherited and hypertrophic cardiomyopathy genetics. This position is critical to providing greater continuity of care to both pediatric and adult patients alike, with special focus on inherited arrhythmias. The HCM genetic counselor will work in complete collaboration with the HCM team and will support programmatic needs as well as strategies aimed at enhancing patient access to care.

  • 1. Obtain and assess patient and family medical and psychosocial histories.
  • 2. Provide patient education, psychosocial counseling, coordinate testing, facilitate decision-making, and make referrals as necessary.
  • 3. Discuss results of diagnostic tests with program team to determine direction of counseling plans.
  • 4. Participate in weekly multidisciplinary case conference.
  • 5. Collaborate with the Hypertrophic Cardiomyopathy (HCM) Physician Leader to coordinate services.
  • 6. Develop written reports on personal and family medical histories and test results.
  • 7. May serve as a Lead Genetic Counselor to an assigned clinic for high risk patients.
  • 8. Participate in institution-wide educational activities as appropriate to role.
  • 9. Identify opportunities for involvement in research, teaching, and community outreach.
  • 10. Travel to outreach clinics as appropriate to support program.
  • 11. Facilitate the HCM patient support group in collaboration with the multidisciplinary team.
  • 12. Assist in the development of the agenda and associated presentations for the annual HCM patient CME event.
  • 13. Work closely with the HCM research team to foster optimal patient participation in applicable research trials.
  • 14. Work in collaboration with the data analyst and researcher to establish metrics and internal benchmarks through which the program can be measured.
  • 15. Assist in the development of a Quality Assurance Performance Improvement forum through which opportunities for improvement can be identified.
  • 16. Implement process improvement measures as appropriate to ensuring optimal patient care.
  • 17. Work in conjunction with marketing and the entire HCM team to optimize marketing of the program and enhance the program's web presence.
  • 18. Able to work autonomously with effective prioritization of job responsibilities.
Qualifications:

EDUCATION


Master's Degree in Genetic Counseling or Human Genetics from an ACGC Accredited Program.


EXPERIENCE


One year of experience as a genetic counselor preferred. Research experience is a plus.


LICENSURE


Board eligibility by the American Board of Genetic Counseling (ABGC) or American Board of Medical Genetics (ABMG) required; certification preferred. Eligible for licensure in the State of Connecticut.


SPECIAL SKILLS


Excellent written and verbal communication skills. Able to work in a team focused approach. Able to work autonomously. Able to lead initiatives paramount to the success of the program and its standing within the broader inherited cardiomyopathies community.
