Job Description

$8,000 Signing Bonus!

Tasked with delivering nursing services to parents and families qualified for the Nurse-Family Partnership (NFP) program, this role is dedicated to fostering health and well-being. Through home visits, it supports low-income, first-time birthing parents and their families from early pregnancy until the child reaches two years of age. Guided by the nursing process and the NFP model of home visitation, it ensures excellence in clinical nursing practice, comprehensive case management, and seamless care coordination—always adhering to the program’s policies, procedures, guidelines, and gold-standard practices. Positions available servicing Bergen & Passaic counties.

ESSENTIAL DUTIES:

  • Provide in-person home visitation to eligible families and carry a caseload of up to 25 clients in accordance with the NFP Model Elements, and guidelines.
  • Complete NFP education and nurse-home visitor competency requirements, including PIPE: Partners in Parenting Education, and ongoing professional development and webinar training.
  • Develop therapeutic relationships and utilize concepts of reflection and motivational interviewing with families in a home visiting environment.
  • Follow nursing process in implementing NFP into nursing practice.
  • Assess the physical, emotional, social, and environmental needs of parents and their families as they relate to the NFP domains.
  • Assist parents and their families in establishing goals and outcomes; provide education, support, and referral resources in attaining goals; evaluate clients’ progress and plan home visits in accordance with clients’ goals and NFP outcomes.
  • Consult and collaborate with other professionals involved in providing services to families.
  • Meet with NFP Nurse Supervisor weekly for clinical supervision and schedule joint home-visit every four months.
  • Attend and participate in team meetings, case conferences, and community events.
  • Maintain HIPAA confidentiality standards and NJ nursing process and standards, code of ethics.
  • Develop and maintain community relationships to support client referrals.
  • Handle other assignments as requested.

JOB REQUIREMENTS:

  • Bachelor of Science in Nursing (BSN) required.
  • Current New Jersey RN license (in good standing) required.
  • BLS certification required.
  • Bilingual language skills in English/Spanish required.
  • Minimum 2 years' experience in maternal/child health, public health, home visiting, or mental/behavioral nursing.
  • Excellent written and verbal communication skills.
  • Ability to work independently & manage client visit schedules to meet program requirements.
  • Strong documentation skills to ensure data entry is properly completed in a timely manner.
  • Strong organizational skills and ability to handle multiple priorities.
  • Basic computer skills and proficiency with Microsoft 365
  • Valid driver’s license and reliable transportation required for regional travel.
